Send your name, date of birth and a list of what you need to sajid.hussain@nhs.net.
Post the right-hand side of your repeat slip to us at 576 Foleshill Road, Coventry CV6 5HP, with a note of your phone number.
Order directly from your GP record with us set as your nominated pharmacy. Nothing to post, nothing to lose.
Order when you have about ten days of medicine left. That covers the surgery, us, and the post — with a few days of slack if something goes wrong.
